The Case Manager Lead provides change-focused interventions to Department of Child Safety (DCS) referred families to achieve Family Connections (FC) core outcomes of: social support, family function, family resources, child well-being, parenting attitudes, and behavior and management of parental stress. The Lead may also provide services for the Nurturing Parenting Program (NPP). NPP works with the family in their homes or temporary residence and assists them to strengthen parental protective factors to reduce the risk of child abuse or neglect and enhance parent/caregiver capacities so that children are safe in their own homes. Case types for both FC or NPP may include family preservation, reunification, or stabilization. The Lead may provide or arrange for transportation of the client, addresses risks of child abuse and/or neglect in client families. Working in communities and travel to family homes is required. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned. ESSENTIAL FUNCTIONS Meet with the referred family once or twice per week in their home or community during the length of the service. Utilize strength-based engagement techniques to encourage participation in services and towards goal achievement. Maintain communication with the assigned DCS Specialist regarding family progress or concerns through email and weekly progress reports. Conduct a comprehensive family assessment utilizing established assessment tools, observation and in person interviews. Facilitate or participate with other stakeholders and the family in meetings to review progress, needs, concerns and barriers on a regular basis per contract timelines. With other team members and family, develop a service plan based on the family's needs, assessment and the risks and safety factors identified by the team and DCS. Assist the family to obtain concrete services, with navigating behavioral health, and other services to support the established goals. Maintain availability to provide services on weekends and evenings when necessary; ensure accessibility to families for emergencies. Complete the Family Nurturing Plan within the first session with the family which is held within 72 hours from the intake meeting. Provide weekly parenting sessions utilizing the Nurturing Parenting curriculum. Thoroughly document all case activity timely and accurately. Submit weekly reports, comprehensive assessment, and service plan timely and accurately. Actively participate as a member of the team. Participate in supervisory meetings with the supervisor, both on an individual basis and as a group; review all new cases with a supervisor. Participate in Child and Family Team (CFT) meetings. Team Decision Making (TDM), Foster Care Review Boards, and other meetings as necessary. Attends meetings as assigned and appropriate. Participate in training and regular process improvement activities. Complete required initial training for Family Connections and Nurturing Parenting and (15) hours of competency-based training per year, to include the Arizona mandated reporter law, cultural competency, how to access Title XIX services, and evidence-based or informed practices. May drive company or personal vehicle on agency business. Must have a Bachelor of Social Work degree or a related Bachelor Degree, with a minimum of one (1) year of human services work-related experience. Must be able to work as part of a diverse group. Must have problem-solving ability. Ability to relate well to families of varied lifestyles, backgrounds, and ethnicities. Experience with multi-problem families. Has or obtains knowledge and understanding of Social Work Theory, Family-Centered Practice, family engagement, motivational interviewing, and treatment philosophies of family-based services. Has or obtains knowledge and ability to access community resources; helps families connect with needed recourses or services. Essential physical requirements include walking, sitting, standing, driving, lifting lightweight office supplies and equipment, talking hearing seeing and writing. REGULATORY Must be at least 18 years of age Valid driver license, proof of insurance, and 39-month motor vehicle report Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card (employer paid) Pass pre-employment drug screen (incudes marijuana, regardless of recreational use laws) (employer paid) DESIRED QUALIFICATIONS Master Degree Social Work, Counseling, or related Human Service field. Additional years of directly related experience in a child welfare setting. Knowledge and understanding of Social Work Theory, Family-Centered Practice, family engagement, motivational interviewing, and treatment philosophies of family-based services. Has or obtains knowledge and ability to access community resources; helps families connect with needed recourses or services. Bilingual, English / Spanish verbal and written.
