Responsible for client assessments, developing, monitoring and evaluating individual care plans, agency outreach and providing referral services to elderly and disabled Deaf and DeafBlind adults at risk of institutionalization. Advocates and provides support services for clients enabling them to extend and enrich independent living through health, financial and social services. Position may from time to time be stressful and require a high demand of performance. May perform other duties as assigned. ESSENTIAL FUNCTIONS Conducts in-home assessment of clients and determines eligibility for services Provides referral of ineligible persons to other resources Explains mail and written English correspondence to consumers, helping them identify scams, respond to formal requests for information or action Develops, implements, and monitors care plans and assists with changes in health status, life adjustment issues, and crises Provides linkage to and advocacy with community and government resources and supportive services Coordinates care plan with client, caregivers, and multi-disciplinary team of providers; Coordinates and facilitates various community education classes Coordinates the emergency/crisis screening process for immediate resolution of crises (such as need for food boxes, emergency shelter, etc) for walk in or scheduled clients Works with consumers to reach or maintain stability, advocating or assisting in eligibility determination for food, housing, utility and transportation funding and referring to appropriate community resources Assists consumers in navigating the eligibility process for government programs, including acute and long term health care Maintains accurate and timely documentation, case records, and ensures contractual compliance Coordinates services with other staff / agencies Participates in consultations, supervision, professional in-services, and training Participates in agency staff, department and network meetings, as well as peer review May drive agency or personal vehicle on company business STANDARD BEHAVIORAL RESPONSIBILITIES Staff members will be respected as human beings with certain job skills who are contributing to the total success of Catholic Community Services Employees should respect each other's workloads and avoid excessive personal conversations causing work to fall behind and work pressures to increase Adhere to Mission and Valued Actions of Catholic Community Services of Southern Arizona, Inc. Meet the needs of internal and external clients with dignity and respect Consistently display positive attitude, and team spirit Accepts responsibility for, and maintains confidentiality Adhere to expectations of required mandatory in-services / certifications / licensures MINIMUM QUALIFICATIONS Suitable work experience may be considered as transferable skills in order to meet minimum requirements of the position, and will be considered by the Corporate Director of Human Resources. BA Degree in related field 2 years of experience in related field Ability to recognize and meet the needs of the population served Proven strong communication skills - oral and written are clear, concise, and in an organized fashion using appropriate style, grammar and tone Conversational skills in American Sign Language REGULATORY Must be at least 18 years of age Valid driver license, proof of insurance, and 39-month motor vehicle report Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card (employer paid) Pass pre-employment drug screen (incudes marijuana, regardless of recreational use laws) (employer paid) DESIRED QUALIFICATIONS MA in human social services or related field Additional years of directly related experience Bilingual, English / Spanish verbal and written Sign Language competency as verified by SLPI, EIPA, RID, BEI or native signer The above statement reflects the general duties considered necessary to describe the principal functions of the job as identified and shall not be considered a detailed description of all work requirements that may be inherent in the job.
