Patient Retention Specialist The Patient Retention Specialist contributes to the mission by planning, coordinating, implementing, and evaluating retention, and re-engagement strategies for out of care patients living with HIV (PLWH). Essential Functions: Work closely with patient's care team with identifying those who have fallen out of care and develop strategies to link them back into care. Conduct retention activities that include, but are not limited to outreach calls, mailing letters, visiting homeless encampments, and patient homes. Facilitate medical transportation services for patients returning to care. Remind patients who are self-managed about medical and lab appointments. Promote communication and collaboration between the clients and all persons involved in the client's care. Follow-up and reschedule medical appointments for patients who miss their appointments. Educate patients on available resources and assist them in accessing those resources. Evaluate and monitor surveillance data to determine patients who are or at risk of falling out of care and work with patient's care team to identify strategies to retain them in care. Maintain community partnerships to coordinate linkage, retention, and re-engagement across systems. Additional Duties Conduct point of care testing amongst people who have never been tested and unaware of their HIV status. Assist with new patient intake and linkage to care activities. Enter data and document patient notes in Epic, ARIES, SHARE or any other data management system. Other duties assigned based on program and organizational need. Other duties assigned based on program and organizational need. Education/Experience Required Required Highschool diploma or GED with at least 3 year's related work experience. California's Driver's License Preferred Bachelor's degree in Social Services or related field 1 year of experience in HIV/AIDS service 1 year of experience providing services amongst LGBTQ and minority/underserved communities.
