The successful candidate will be required to work 37.5 hours per week, working across Monday - Sunday, on a rota basis. Shifts will vary between 10 hour, 8 hour and 5 hours shifts between 9AM - 8PM. Cover is ongoing on a rolling monthly basis. In this position, you will be expected to; - Motivate and coach clients to attend appointments and engage in activities. In this position, you will be expected to; - Motivate and coach clients to attend appointments and engage in activities. - Work to ensure the maximisation of income for client and the service, including the regular review of benefits for clients. - Record needs and risk backssments onto the systems- Complete referrals & attend and advocate at move on backssments. - Support clients with the processes of moving into the scheme. - Ensure all practical issues in relation to the accommodation are addressed such as pre-screen for risks. - Identify information to fully inform the backssment and support planning process- Support with the management of rent/service charge payments, attending statutory appointments in order to comply with treatment or criminal justice requirements- Assist clients to participation in activities to promote increased independence and involvement in the community. - Ensure information necessary for clients to be successfully referred into other services is available and forwarded on. - Through coaching and 1:1 support, build the clients abilities and confidence to carry out tasks for themselves, focusing on the skills that support independent living. - Establish and maintain good links with local statutory and voluntary agencies and other community resources. - Develop and broker opportunities for clients with individual.
