About the Service / Department Run over three properties, Hammersmith & Fulham's South Cluster provides two of which for individuals with low-medium mental health requirements and the third for service users with greater needs. The services all support adults (18+) with enduring mental health needs, who require additional housing and other support. This service is for users who have a recognised need for a level of supported accommodation who may be leaving hospital, registered cared or at risk of homelessness. The service is used as a ‘step-up' or ‘step down' from other supported accommodation, where appropriate. The South Cluster aims to equip service users with the skills to move on and live independently in the future. About the role Manage a caseload of Service Users' with varying degrees of mental ill health, and support needs and their carers to manage their mental health and wellbeing, maintain their independence, identify their strengths and achieve their goals and aspirations Have some supervisory responsibility for staff/students within the team Support with a range of medications within protocol and within current Hestia policy, and document this accurately Willingness to work unsociable hours within a rota system including weekends, evenings Housing, Community care, CPA Health and Safety and DWP legislation To clearly articulate the aims and objectives, pathway and service model to people using the service so they can fully participate in and co-produce their support Empower service users to engage effectively in their liaisons with professionals within the community such as social work teams, housing providers, health professionals About you Ability to work with people in a person-centred way An understanding of the support needs of older adults Good Literacy and numeracy skills. Working knowledge of basic functions in MS Word and Outlook or willing to learn Excellent Knowledge of issues related to signs, symptoms and treatments. Visit ListedBuy for more information.
