Our services include emergency food transitional housing and education for homeless and street-involved youth; hospice palliative care; community mental health programs; and supportive housing programs. By integrating a Harm Reduction framework at Good Shepherd, we can decrease barriers to service and better support some of our community's most marginalized members. This opportunity is with our Good Shepherd - Men's Services Good Shepherd is seeking experienced Registered Practical Nurse interested in providing excellence in care in a community setting. The Good Shepherd Transition Beds program provides brief-stay residential care to marginalized individuals being discharged from hospital who require ongoing medical care and case management services to support optimal recovery and re-integration into the community. How You Will Make an Impact You are a healthcare professional interested in working in a dynamic community setting responding to the needs of people impacted by poverty, homelessness, mental health issues and substance use You will be part of a multi-disciplinary team that will facilitate improved health status and timely, effective discharge to community for service users What You'll Bring to Our Team Registered Practical Nurse in good standing with the College of Nurses of Ontario and currently authorized to practice in the Province of Ontario A current Certificate of Competence form the College of Nurses of Ontario Demonstrated ability to work with individuals with diverse and complex health and psycho-social needs Strong backssment,.
