The RN Case Manager performs the primary function of a Registered Nurse in assessing, planning, implementing, and evaluating the care of patients in the Wound Healing Center. Assess, stage, and measure wounds accurately and in accordance with policy. Assists physician with wound therapy. Performs assessments and documents ongoing care plans. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Provides patient education.
